Testosterone Therapy and the Value of a Precise Diagnosis
Testosterone therapy can be useful for men with a confirmed hormone deficiency, but feeling tired or getting older does not establish that diagnosis. The strongest starting point is understanding what has changed, what may be causing it, and which outcomes matter to you. For adults in Metairie, New Orleans, and the Gulf South, that approach can prevent months of chasing a laboratory number while the real problem remains untreated.
At Infinite Health, hormone-related concerns belong within a broader discussion of health. Testosterone may be one component of an individualized plan when there is a sound clinical reason to use it. It is not a universal prescription for energy, weight loss, or longevity.
When testosterone replacement can add value
In men, testosterone supports sexual development and function, muscle, bone, and red blood cell production. FDA-approved replacement products treat deficiency associated with specified medical conditions affecting testosterone production. Their labeling retains limitations regarding low testosterone attributed to aging alone. FDA testosterone labeling update.
Symptoms can include reduced sexual desire and changes in spontaneous erections, but fatigue and low mood have many possible explanations. The Endocrine Society recommends compatible symptoms or signs plus consistently low, accurately measured testosterone, generally confirmed with repeat morning fasting testing. Further evaluation should identify the cause. Endocrine Society clinical guideline.
An appropriate benefit discussion is specific: Which symptom is likely hormone-related? What improvement is realistic? How will we decide whether continuing treatment is worthwhile? An increase in the blood level, by itself, does not answer those questions.
The evaluation should look beyond the hormone result
A useful visit includes medication and supplement review, sleep, alcohol use, weight changes, medical history, and fertility plans. Obesity, certain medications, and other reversible contributors may affect both symptoms and testosterone. The Endocrine Society's 2026 statement emphasizes accurate diagnosis and addressing these contributors; weight management is typically first-line when excess weight is the identified cause. 2026 prescribing statement.
Testing is selected to answer clinical questions. Depending on the situation, that can include repeat testosterone, assessment of binding proteins or free testosterone, and hormones that help distinguish testicular from pituitary causes. A broad panel or an advertised “optimal” range cannot substitute for interpretation.
Fertility and safety deserve an early conversation
External testosterone can suppress sperm production. Someone hoping to conceive should discuss that goal before treatment, rather than assuming replacement improves fertility. Elevated hematocrit, untreated severe sleep apnea, prostate or breast cancer, certain prostate findings, recent heart attack or stroke, and other conditions can change eligibility or require additional evaluation. Endocrine Society clinical guideline.
Monitoring depends on the formulation and individual. It commonly includes symptom response, testosterone exposure, blood count, blood pressure, and an appropriate prostate discussion. Topical gels also require precautions to prevent transfer to other people. Product-specific instructions matter because formulations are not interchangeable. FDA Testim prescribing information.
What newer heart safety evidence actually tells us
The TRAVERSE trial studied 5,246 men with symptoms, repeatedly low testosterone, and existing or elevated cardiovascular risk. Testosterone gel was noninferior to placebo for the trial's primary major cardiovascular outcome. Atrial fibrillation, acute kidney injury, and pulmonary embolism occurred more often in the testosterone group. The study was funded by testosterone manufacturers and does not establish safety for every formulation, population, or duration. Original TRAVERSE trial.
In 2025, FDA removed the class-wide boxed-warning language about increased adverse cardiovascular outcomes while requiring blood-pressure warnings. These changes support a more precise conversation; they do not turn testosterone into a cardiovascular prevention treatment. FDA update.
Building a plan that works outside the office
Infinite Health's personalized health and longevity approach connects diagnostics and hormone assessment with nutrition and metabolic health, physical fitness, mindset, and appropriate regenerative medicine evaluation. These pillars provide different tools for different problems; every patient does not need every intervention.
For someone whose fatigue tracks with disrupted sleep and declining conditioning, improving those areas may deserve priority. For someone with established hypogonadism, an appropriate prescription may sit alongside progressive strength training, adequate nutrition, and a workable follow-up schedule. Regenerative options require their own indication and evidence review; they are not necessary partners to testosterone.
The aim is a plan you can evaluate in daily life, including sexual function, activity tolerance, and treatment burden, without promising that replacing a hormone will reverse aging.
Frequently asked questions
Will testosterone reliably fix low energy
No. For age-related low testosterone, the American College of Physicians advises against initiating treatment to improve energy, vitality, cognition, or physical function because evidence does not support those uses. A clinician should investigate other explanations. ACP guideline summary.
Is testosterone ever considered for women
Yes, in a different clinical context. International consensus supports consideration for carefully assessed postmenopausal hypoactive sexual desire disorder, rather than a low blood level alone. U.S. prescribing for women is off-label, and long-term safety remains uncertain. Acne and increased facial or body hair are possible and require monitoring. It is not established treatment for general wellness or disease prevention. Global consensus statement.
Can I start the discussion through telehealth
Ask about availability. Eligibility depends on your location, applicable prescribing and licensing rules, clinical appropriateness, and any needed examination or testing. A virtual visit does not guarantee a testosterone prescription.
