Bioidentical vs. Synthetic Hormones: What’s Actually the Difference?

“Bioidentical” sounds natural. “Synthetic” sounds artificial. Put the two words side by side, and it is easy to assume that one is inherently safer, gentler, or better for your body.
That is not how hormone therapy works.
The distinction between bioidentical and synthetic hormones is frequently blurred by marketing language. A hormone can be bioidentical and manufactured in a laboratory. A medication can be bioidentical and FDA-approved. It can also be bioidentical and custom compounded. Those categories describe different things—and they should not be treated as interchangeable.
The most useful question is not simply, “Is this hormone natural or synthetic?” It is: What is the exact hormone, why is it being prescribed, how will it be delivered, what evidence supports it, and is it appropriate for me?

What Is a Bioidentical Hormone?
A bioidentical hormone has the same chemical and molecular structure as a hormone produced by the human body.
Examples used in menopausal hormone therapy include:
17-beta estradiol, which is structurally identical to the primary estrogen produced by the ovaries during the reproductive years
Micronized progesterone, which is structurally identical to human progesterone
The word bioidentical describes molecular structure. It does not tell you whether the medication is:
FDA-approved or compounded
Made by a large pharmaceutical manufacturer or a compounding pharmacy
Available by prescription or improperly marketed over the counter
Delivered as a pill, patch, gel, spray, cream, insert, or another form
Safer or more effective for a particular patient
That last point is important: molecular similarity by itself does not establish safety.
What Does “Synthetic Hormone” Mean?
Synthetic is an imprecise term in conversations about hormone therapy.
Sometimes people use it to describe any medication manufactured or processed in a laboratory. Under that definition, FDA-approved estradiol and micronized progesterone could be called synthetic even though their finished molecules are bioidentical.
Other times, synthetic hormone is used to describe medications that act on the same receptors as human hormones but do not have precisely the same molecular structure. Examples can include certain progestins and conjugated estrogens.
This is why “bioidentical versus synthetic” is not a clean scientific comparison. A more accurate discussion identifies the exact estrogen or progestogen being considered.
Bioidentical Does Not Mean Compounded
This is perhaps the biggest source of confusion.
Bioidentical hormone medications can be divided into two broad categories:
FDA-approved bioidentical hormones
The FDA has approved multiple products containing hormones identical to those made by the body, including estradiol and micronized progesterone. These products are available in standardized doses and multiple delivery forms.
FDA approval means the product has been evaluated for:
Safety and effectiveness for its approved use
Dose consistency
Manufacturing quality
Purity and stability
Labeling and required safety information
FDA-approved does not mean risk-free. It means the product has gone through a defined review and quality-control process.
Compounded bioidentical hormones
Compounded medications are prepared for an individual patient by a compounding pharmacy. They may be useful when a patient has a documented need that cannot be met by an available FDA-approved product—for example, an allergy to a particular inactive ingredient or a required dosage form that is not commercially available.
However, compounded hormone preparations are not FDA-approved. The FDA does not evaluate each compounded formulation for safety, effectiveness, absorption, or dose consistency before it reaches the patient.
Major medical organizations do not recommend routinely prescribing compounded bioidentical menopausal hormone therapy when an FDA-approved formulation can meet the patient’s needs. Compounded does not automatically mean more personalized, more natural, or safer.
Are Bioidentical Hormones Made From Plants?
Many bioidentical hormones begin with plant-derived raw materials, often compounds found in soy or wild yam. Those plant compounds must be chemically processed in a laboratory to create pharmaceutical estradiol or progesterone that is structurally identical to human hormones.
Your body does not convert an over-the-counter wild-yam cream into prescription progesterone on its own.
“Plant-derived” may sound appealing, but the source material does not determine whether the final medication is safe or appropriate. The identity, purity, dose, route, evidence, and manufacturing controls matter far more.
Estrogen, Estradiol, Progesterone and Progestin: A Quick Translation
Hormone terminology can feel needlessly complicated. These distinctions help:
Estrogen
Estrogen is a category of hormones rather than one single substance. Estradiol, estrone, and estriol are all estrogens.
Estradiol
Estradiol is the primary estrogen during the reproductive years. Pharmaceutical 17-beta estradiol is bioidentical.
Progesterone
Micronized progesterone is chemically identical to the progesterone produced by the body. “Micronized” means it has been processed into smaller particles to improve absorption.
Progestin
Progestins are compounds that produce progesterone-like effects but are not necessarily structurally identical to human progesterone. Medroxyprogesterone acetate is one example.
Progestogen
Progestogen is the umbrella term that includes both progesterone and progestins.
For a woman who still has a uterus and uses systemic estrogen, adequate endometrial protection with an appropriate progestogen is generally essential. Estrogen without sufficient protection can stimulate the uterine lining and increase the risk of endometrial hyperplasia and cancer.
Are Bioidentical Hormones Safer Than Non-Bioidentical Hormones?
It is not medically accurate to declare all bioidentical hormones safer than all non-bioidentical hormones.
Risk can depend on:
The specific hormone and formulation
Oral versus transdermal delivery
Systemic versus local treatment
Dose and duration
Whether estrogen is properly paired with endometrial protection when required
Age and time since menopause began
Personal and family medical history
History of breast cancer, uterine cancer, blood clots, stroke, heart disease, liver disease, or unexplained bleeding
For example, the route of estrogen matters. Oral and transdermal estrogen are processed differently, and transdermal delivery may have a different blood-clot risk profile than oral estrogen for some women. That is a route-of-administration issue—not simply a bioidentical-versus-synthetic issue.
The safest option is the one selected after an individualized benefit-risk assessment and prescribed in an evidence-based dose and form.
Do Bioidentical Hormones Work Better?
Bioidentical hormones can be effective for appropriate menopausal symptoms. Systemic estrogen is the most effective treatment for bothersome hot flashes and night sweats. Local vaginal estrogen can treat certain vaginal and urinary symptoms associated with menopause.
But there is no credible basis for claiming that compounded bioidentical hormones work better than FDA-approved bioidentical products as a category. The FDA and The Menopause Society state that compounded products have not been shown to be safer or more effective than approved bioidentical hormones.
Effectiveness should be judged by the symptom being treated, the exact product, and the patient’s response—not by marketing terminology.
What About Saliva Testing and “Custom” Hormone Formulas?
Some compounded hormone programs use saliva testing to create a supposedly precise custom dose. This approach sounds scientific, but hormone levels can fluctuate, and a single saliva measurement generally does not provide a reliable basis for individualizing menopausal hormone therapy.
Good personalization is broader than a laboratory number. It considers:
Your most disruptive symptoms
Menstrual and menopause history
Sleep, mood, sexual, vaginal, and urinary concerns
Uterine status
Medical and family history
Current medications and supplements
Cardiovascular, metabolic, breast, and bone-health factors
Response and side effects after treatment begins
Laboratory testing may be appropriate in selected circumstances, but treatment should not become a race to achieve a universal “optimal” hormone level.
How Should You Compare Your Options?
Instead of asking only whether a hormone is bioidentical, ask your clinician:
What is the exact name of each hormone being prescribed?
Is the product FDA-approved or compounded?
If it is compounded, why can an approved formulation not meet my needs?
What symptom is each hormone intended to treat?
What benefits are reasonably supported by evidence?
What risks apply to me personally?
Why are you recommending this delivery method?
If I have a uterus, how will the uterine lining be protected?
Which side effects or bleeding changes should I report?
When will we reevaluate the treatment?
A clinician should be able to answer these questions without hiding behind “natural,” “custom,” or “anti-aging” language.
The Bottom Line
Bioidentical and FDA-approved are not opposites. Neither are bioidentical and prescription. Estradiol and micronized progesterone can be bioidentical, commercially manufactured, FDA-approved prescription medications.
Likewise, compounded and personalized are not synonyms. True personalization comes from choosing an appropriate therapy based on the patient—not simply creating a custom mixture.
For some women, menopausal hormone therapy can make a meaningful difference in hot flashes, night sweats, sleep disruption, vaginal symptoms, and quality of life. The decision should focus on the right patient, hormone, dose, route, and clinical purpose.
